A Happy Family We Are

FCSN Supported Living Services (SLS) assists you to live on you own with support.  24/7 services can be provided if needed and approved by the Regional Center.  FCSN SLS Program is tailored to meet your needs, skills and interests for a successful and fulfilling life. You decide where to live, with whom, what support needed and how to live healthily, happily and safely.

FCSN SLS conducts Person-Centered Planning with you and your circle of support, assists you with affordable housing search, provides thorough living skills training and support, and helps you engage in life-enrichment and leisure activities in the community.

Living Skills Training

  • Self care skills and hygiene

  • Meal planning, grocery shopping, cooking

  • Housekeeping skills

  • Communication, social skills

  • Effective conflict resolutions

  • Recreation and leisure skills

Health and Wellness

  • Healthy/balanced meal preparation

  • Healthy lifestyle practices

  • Scheduling and attending annual or as needed medical, dental, vision, OB/GYN, psychiatric appointments

  • Medication Handling

  • COVID mitigation practices

Budgeting & Monitoring

  • Affordable housing and compatible housemate(s)

  • Budgeting and timely paying bills

  • Financial monitoring

  • Public benefits (Cal Fresh, SSI, SSDI, IHSS, Section 8 Housing, Medi-Cal, Medicare)

 

 

Safety and Emergency Planning

  • Individualized emergency preparedness and training

  • Safety awareness and practices

  • Drills- Fire, Earthquake, Power outage, Target violence

  • Public and community emergency resources

 

SLS Five Principles

  • 18+ years old

  • A Regional Center client

  • Financially able to pay for housing & all living expenses

  • Desire to reside in south Alameda County or Santa Clara County.

  • Without complicated health issues

  • No aggressive behaviors toward self or others
